Public Inspection of Documents Policy
The following organizational and governing documents of the Pancreatic Cancer Action Network will be available (for inspection or copying) at the organization’s main office during normal business hours at no charge:
Tax Exemption Application (Form 1023)
Internal Revenue Service Determination of Exempt Status
Articles of Incorporation
Conflict of Interest Policy
All of the aforementioned documents will also be posted on the organization’s web site. The organization will make best efforts to ensure that the documents posted on the organization’s web site are the most updated versions of such documents (e.g. in the situation where the organization’s Articles of Incorporation are revised).
The following financial information from the previous three years will be available (for inspection or copying) at the organization’s main office during normal business hours at no charge:
All of the aforementioned financial documents will also be posted on the organization’s web site. Beginning with the fiscal year ended June 30, 2009, the public inspection copy of the Form 990 will not include Schedule A, schedule of excess contributors or Schedule B, names and addresses of contributors. For fiscal years June 30, 2008 and prior, Schedule B is similarly excluded.
The organization will make best efforts to ensure that the Forms 990 held at their main office and posted on their web site are the most updated versions of such (e.g. in the instance where a Form 990 has been amended). When responding to a public inspection request for any organizational, governing or financial document by anyone, the organization shall fulfill such request in a timely fashion without inquiring as to the reason for the public inspection request.
